I am the owner or authorized agent of the pet described below and voluntarily request creative grooming services, including the application of professional pet-safe creative color products. 

 

I understand creative grooming is a cosmetic, artistic service and that color is semi-permanent, typically lasting around 2 to 5 months, and that it will fade gradually, sometimes shifting through different shades. 

 

I acknowledge that every pet is unique and that factors such as coat texture, density, oils, and health can all affect a color outcome. Some areas may be lighter or darker, or less vibrant, and that is not considered a defect in application. No guarantee is made regarding exact shade, brightness, or longevity. 

 

Puppy Love Spa reserves the right to refuse or discontinue creative grooming if it is deemed not in the pet's best interest, including for elderly or medically compromised pets. 

 

I understand that bathing Swimming, medicated shampoos, sun exposure, and normal coat growth may accelerate fading and maintenace requires future services. 

 

I acknowledge that creative grooming is a artistic service and minor variation are normal, and not grounds for refunds OR repeat application.  

 

I'll authorize Puppy Love Spa to photograph my pet for records and marketing purposes unless declined in writing. 

 

I confirm that the requested colors and design were reviewed with me before the service, and I approve the plan. 

 

I acknowledge that I have had the opportunity to ask questions and that I voluntarily consent to the requested services and release Puppy Love Spa from liability arising solely from normal fading, variation, or uneven absorption due to my pet's individual characteristics and natural wear over time.
