1. Consent: I am the owner or authorized agent of the animal described above. I have the authority to execute this consent and I am over the age of 18.
2. Vaccination: To my knowledge my pet is up to date on all required vaccines including Rabies, is healthy, and free of disease.
3. Service: I hereby consent to and authorize the performance of requested procedures and grooming services to my pet. I understand the risks that may be involved.
4. Fee: I agree to pay all fees for requested services and procedures at the time of discharge.
5. Punctuality: Unless alternate agreements have been made, If I do not pick up my animal within 1 hour of the pickup time, I agree to pay a $10/hr hotel fee for the storage and supervision of my animal. Additionally I agree to pay the cancellation fees set out in the Puppy Love Policies if I fail to cancel an appointment within 24 hours.
6. Correspondence: I agree to have texts sent to me regarding appointment confirmation, pickups, and other correspondence with respect to my pet.
7. Right to Refuse Service:The groomer has the right to refuse service if the pet is extremely aggressive or if the groomer feels that it is unsafe to perform any services on the animal.
Matted Pets: Sometimes undercoat and hair removal becomes necessary when a coat is tangled beyond the ability to brush through it. The best and most humane method to resolve this issue is to shave off the matted hair. Shaving the hair is the least stressful and least painful way to groom a matted pet.
By booking a grooming appointment, you agree to a dematting fee of $20 for every 15 minutes required.
Some pre-existing conditions may be revealed after de-matting. Some conditions that may reveal themselves are:
• Matting can pull on skin and cause bald spots and abrasions
• Increased heat and an environment prone to bacterial/fungal infections
• Working so close to the skin may cause irritation – All precautions will be taken to minimize this.
• Mats create an ideal environment for parasites such as fleas and ticks. If either are found, the grooming process will stop and the parent will be contacted to pick up their pet.
• Ear bruising (hematomas) due to excessive head shaking after removal of mats
• Sunburn if your dog is kept outside for long periods of time after removal of mats. Prevention includes pet sunscreen, skin coverage (clothing) and reduced time outside from 10 am to 3 pm
• To avoid matting your pet should be brushed and combed on a regular basis
8. Matting Risk: If my pet has matted hair, I understand these risks and authorize the dematting process of my pet, and to shave off any and all matted hair as necessary. I understand the hair may be cut shorter than I wish. Should my pet need any veterinary care during or after this process, I agree to pay any and all veterinarian fees.
9. Medication: If your Pet is currently or in the last 30 days been on any sedatives or medications, I take full responsibility and liability for any form of reaction or consequence as a result of my pet being on medication. I agree to be refused service by the business for any reason as a result of the medication of my pet.
10. Indemnification: Client will hold harmless and indemnify 5018261 Ontario Ltd. from and against any losses, damages, expenses, fines, monetary penalties, liabilities claims and demands of any nature or kind including any interest and all legal fees and costs on a substantial indemnity basis (each a “claim”) which it may suffer or incur by reason of any breach or allegation of breach of any warranty, representation or agreement made by Client under this Agreement.
11. Policies: I have read and understand the Puppy Love Policies.
