By submitting this form, I confirm that I am the owner or authorized agent of the pet described above. I am over 18 years of age and have full authority to provide this consent on behalf of the pet’s legal owner.
I hereby authorize Puppy Love Spa to take all reasonable actions necessary to ensure the health, safety, and well-being of my pet during their stay. I understand and agree that I am solely responsible for all costs incurred during the boarding period, including any medical treatment, special care, or supplies required.
Late Pick-Up Policy
A late fee of $10 per hour will be applied if the pet is picked up after the scheduled time.
Failure to Pick Up & Abandonment Policy
If the pet is not picked up at the agreed-upon time, the following steps will be taken:
- Day 1: Puppy Love Spa will attempt to contact the Owner via phone, text, and email. If unreachable, the Secondary Contact listed on the form will also be notified.
- Day 2: A formal written notice will be issued to both the Owner and Secondary Contact, advising that the pet must be picked up within 24 hours. Additional boarding fees will apply.
- Day 3: If no response is received, and the pet remains unclaimed, the pet may be considered abandoned. In accordance with the Ontario Provincial Animal Welfare Services Act, the pet may be surrendered to a shelter, rescue, or rehomed at the Facility’s discretion.
Abandonment Clause
The Owner acknowledges that failure to retrieve their pet constitutes abandonment under Ontario law. Once the pet is surrendered or rehomed, Puppy Love Spa is released from any further responsibility or liability.
Liability for Costs:
The Owner remains financially responsible for all outstanding boarding and care fees. Legal action may be taken to recover these costs, if necessary.
Special Circumstances Clause
In cases of emergency, medical incidents, or travel delays, the Owner or Secondary Contact must notify Puppy Love Spa within 24 hours of the scheduled pick-up time to make alternate arrangements. Additional fees will apply based on current rate schedules.
Additional Terms
- I understand and agree to pay for any special care, food, or services provided during my pet’s stay, if required.
- I accept full responsibility for any damage or injury caused by my pet while on the premises due to aggressive, destructive, or inappropriate behavior.
- I consent to allow my pet to socialize with other pets. I understand that minor scratches, nicks, or play-related abrasions may occur, and I assume responsibility for any such outcome.
- To the best of my knowledge, my pet is healthy, free from contagious conditions, and current on all required vaccinations, including rabies.
- I consent to receive text messages from Puppy Love Spa regarding appointments, pick-ups, and pet-related updates.
- I acknowledge that I have read, understood, and agree to all Puppy Love Spa policies as outlined on the website and in this agreement.
Emergency Veterinary Care & Credit Card Authorization
The Owner agrees that a valid credit card must be kept on file with Puppy Love Spa at all times while the pet is in the care of Puppy Love Spa, including during daycare, boarding, or any extended stay. The Owner is responsible for ensuring that the card remains valid and has sufficient available funds to cover any charges that may arise in connection with the pet’s care.
In the event of an illness, injury, medical emergency, or other situation in which Puppy Love Spa reasonably believes veterinary attention is necessary, Puppy Love Spa will make reasonable efforts to contact the Owner and/or Secondary Contact using the contact information provided.
If the Owner or Secondary Contact cannot be reached, the Owner authorizes Puppy Love Spa to transport the pet to a veterinarian or emergency veterinary facility and to authorize veterinary examination, diagnostic testing, treatment, medication, hospitalization, or other care that is reasonably considered necessary to protect the pet’s health, safety, or well-being.
The Owner acknowledges and agrees that all veterinary fees, emergency medical expenses, medications, diagnostic testing, hospitalization, transportation costs, special-care expenses, and other related costs incurred on behalf of the pet are the sole financial responsibility of the Owner.
The Owner authorizes Puppy Love Spa to charge the credit card on file for costs incurred by Puppy Love Spa in connection with such emergency veterinary care, including amounts Puppy Love Spa is required to pay or advance on the Owner’s behalf.
The Owner understands that Puppy Love Spa’s decision to arrange or facilitate emergency veterinary care does not mean that Puppy Love Spa has accepted financial responsibility for that care.
