I certify that I am the client receiving services or the legal parent, guardian, or authorized representative, and I voluntarily consent to the chosen wellness services. Health disclosure. I certify that all information provided is complete and accurate to the best of my knowledge.
Nature of wellness services. I understand these services support relaxation and well-being, and are not a substitute for medical or veterinary care. Client responsibility. I will inform my practitioner of discomfort and may request modifications or end the session at any time.
Assumption of risk. I understand participation may involve temporary soreness, fatigue, emotional responses, or aggravation of pre-existing conditions, and I voluntarily accept these risks. Release of liability. To the fullest extent permitted by law, I release and hold harmless Puppy Love Resort Wellness Centre, 5018261 Ontario Ltd., its owners, employees, contractors, and practitioners from claims arising from my participation, except where liability cannot be excluded by law. Emergency authorization. If an emergency occurs and staff believe immediate medical or veterinary attention is needed, I authorize them to seek such care on my behalf.
right to refuse. I understand the Wellness Centre may refuse, modify, postpone, or discontinue services to support safety and well-being.
Privacy. I consent to the collection and use of my personal information for appointment scheduling, communication, record keeping, and the delivery of Wellness Center services. Photo and media consent. Check one. I consent to photographs or videos being used for educational or promotional purposes, or I do not consent.
Deposit and cancellation policy. I understand that a 50 dollar deposit is required to reserve my appointment and will be applied toward my scheduled service. I understand that a minimum of 72 hours notice is required to cancel or reschedule. Appointments cancelled with less than 72 hours notice, missed appointments, or no shows will result in forfeiture of the 50 dollar deposit. Client acknowledgement.
