I acknowledge that Puppy Love has advised me that my pet is a high-anxiety, high-risk and or aggressive grooming client. *

I acknowledge that my pet may suddenly move, jerk, jump, twist, pull away, struggle, kick, bite, snap or resist handling, creating increased risk. *

I acknowledge that medication or veterinary anxiety management has been discussed with me as recommended or required." Initials line. *

I understand that failure to follow medication or safety instructions may significantly increase the risk of injury. *

Cut or Nick to the body *

Ear cut or injury *

Tongue injury *

Lip or mouth injury *

Eye area may be left longer for safety *

Face or muzzle may not be fully groomed for safety *

Nail bleeding or quicking *

Incomplete nail trim or grind *

Hematoma *

Bruising, swelling, or redness *

Skin rash or irritation *

Matted coat may conceal pre-existing sores or skin issues that become visible only after grooming *

Sanitary or sensitive areas may be modified or discontinued for safety *

Incomplete or partial groom *

Groom may not match requested style, length, or finish *

Uneven or modified finish due to safety *

Face, ears, feet, or nails may not be fully finished *

Safety takes priority over appearance *

I authorize the groomer to modify the groom or stop immediately if unsafe, acknowledging completion is not guaranteed. *
